A personal visit to the care home so we can meet with your loved one and key staff members.
Full Care Review
A full review of your loved one’s care plan and risk assessment to ensure their needs are thoroughly understood and met.
Independent Report
An independent opinion on the adequacy of care provided and recommendations for any necessary changes or improvements.
Direct Communication
Direct communication with family and care home staff to ensure all parties are informed, engaged, and on the same page.
Findings Discussion
Follow-up support with a Zoom call to discuss findings and recommendations and to collaboratively plan the next steps for your loved one’s care.
Further Support
Option to have extra support during a review meeting with care home staff on the day of the assessment at no additional cost.
